If you've ever looked a the quality of records from a project that has a document controller on it, you should have been impressed - I know I have been frequently. Beautifully arranged, perfectly titled, complete. Records that really are capable of telling the story, saving our behinds, and helping us spot anything untoward.
Yes - I've seen RACI matrices, but only once or twice in RM and generally around project responsibilities where IT were involved (and had a good service management practice). The consequences aren't obvious things - they're the bits of friction that no one notices, even when they're putting a 40 to 50% tax on productivity.
If everyone is responsible then no one is responsible? Two thoughts:
- Have you seen responsibility matrices used in RM (e.g. RACI) for different functions / domains?
- What are the consequences for not adequately undertaking your responsibilities?
